I didn’t know Dolly, but got to meet her and other Grand Ole Opey stars during the Fall of 1983. I worked summers at Opryland during college and for 1 Fall I was an usher at the Opry. She was tiny (5’ if she was lucky) but had the biggest smile and personality of them all. She would take the time, when she saw us knucklehead ushers, to come over and say hello and thank us. Thoughtful and down to earth. Most of the country stars were down to earth back then (e.g. Stringbean, Grandpa Jones, Roy Clark, Roy Acuff, Barbara Mandrell, Patsy Cline, Tammy Wynette, … and of course Dolly). Fun times. The best job I had (even @ $3.75 an hour) other than flying in the AF.

I would highly encourage you to just strictly submit what the PDSM wants. I have buddies that got selected in the last board, no Form 8s in their app their 942 was clean. I also knew someone (he flies KC-135 now) who applied with a Q3 on his record and he submitted the applicable Form 8 that correlates with the one on his 942.

Maybe I’m alone in this but I thought the application itself was rather large (so to speak), and was scrambling to make sure everything was there. Maybe this a rated to rated thing but I ended up attaching all my Form-8s and the eval form (whatever it’s called AF-9something). What attachments do you have? DAF215, Attachment 1 (letters, rated eval forms), Certified Physical, ADSC Statement, and PPL copy is all I've got

That's how I'm thinking. OTS package was far larger and included every non-medical record. Whereas this is just a handful of documents I read the form 8 section as "include it because we want to see an explanation for Q3s, even if you don't have any. It's required for rated applicants". I may just be over thinking since the application is so small

Criminally insane, years on psych ward and with full support of family was at the point of starting stages of community release. Doctors treating him fully admitted that if he stops taking his meds, it would take 2-5 days before his psychosis kicks back in.
