You don’t need to send OPB or 475. They pull RoP themselves. I don’t think that’s how it read, I see what you mean. They want a justification for a Q3 and a form 8 will show that. I would highly encourage you to just strictly submit what the PDSM wants. I have buddies that got selected in the last board, no Form 8s in their app their 942 was clean. I also knew someone (he flies KC-135 now) who applied with a Q3 on his record and he submitted the applicable Form 8 that correlates with the one on his 942.

DAF215, Attachment 1 (letters, rated eval forms), Certified Physical, ADSC Statement, and PPL copy is all I've got

That's how I'm thinking. OTS package was far larger and included every non-medical record. Whereas this is just a handful of documents I read the form 8 section as "include it because we want to see an explanation for Q3s, even if you don't have any. It's required for rated applicants". I may just be over thinking since the application is so small

Dad came home one day to his son standing by his wife/mother who the son had smashed in her skull. Criminally insane, years on psych ward and with full support of family was at the point of starting stages of community release. Doctors treating him fully admitted that if he stops taking his meds, it would take 2-5 days before his psychosis kicks back in.

You did not need to attach a Form 8 unless you had a Q3 on your FEF. That’s why you just attach a 942. I think the application is small but this is coming from a guy who was Prior E and applied to OTS where it was a ton of paperwork.

It’s normal, if you don’t have any waivers or other things like ETP. They pull your records from PRDA. Make sure all your records are input. My buddy who got selected for ENJJPT was missing his URT 475 and they called to tell him while the board was ONGOING! Luckily they let him track it down and he got selected for ENJJPT still. Make sure anything that says DG too is in record.
