I think it has more to do with yesterday’s leaders picking HPOs who conform to their norms, who in turn are not necessarily the best equipped to lead their peers and subordinates.

The irony is that there are probably plenty of articles out there with far more details about weapons drops and shooting terrorists that are still considered "just fine" by AF PA.
