188th UPT Board slated for Feb 2015

In order to qualify for Undergraduate Pilot Training (UPT) selection, please compile the following information into a concise package. Packages must be received by this office before close of business 9 January 2015. The Selection Board will be held here at the 188th on 7 February 2015. Please note that travel to the interview is at the candidate's expense. Hotel rooms are provided. Package requirements are as follows:

A AF Form 883 (signed in lower left corner)

B Head to Waist 8” x 10” Photograph

C AF Form 24

D Resume

E Copy of Birth Certificate

F Air Force Officer Qualifying Test (AFOQT) scores

G One consolidated college transcript (unofficial acceptable)

H Copy of College Diploma

I Letters of recommendation (5 maximum)

J DD Form 2807-2

K 188 FW/DPR Height/Weight Verification Letter

L AF Form 2030

M Basic Attributes Test (BAT) scores & PSCM scores

N Private pilot’s license

O Copy of last page of log book

P Department of Motor Vehicle drivers record (current within 90 days)

Q DD Form 214/NGB 22 (Prior Service applicants)

2. Please include a loose-leaf, unstapled, single-sided 8 1/2 X 11 copy of your package for ease of photocopying. If possible, an electronic copy of application submitted via physical medium (CD or DVD).

3. This information will only be used to determine whether a candidate meets the minimum criteria for the interview process. All packages become the property of the Arkansas Air National Guard to be retained or disposed of at our discretion.

4. The top applicants will be called for a formal interview. The Undergraduate Pilot (UPT) selection board will then select an undetermined number of MQ-9 pilots. Alternates from prior boards are welcome to resubmit an application package. If a primary candidate becomes unable or ineligible to attend UPT, then an alternate will fill the slot.
